The CISSS de la Montérégie-Ouest is among the institutions designated to take part in the provincial pilot project to open clinics for patients with long COVID and chronic Lyme disease.
The services for long COVID patients introduced at the CISSSMO in 2021 have been revised to reflect the parameters of the government project. The services provided by the team are mainly virtual (via video calls).
Specialized rehabilitation services - long COVID
The services for chronic Lyme disease will be developed over the next few months.
Make a service request
If you have persistent symptoms or after-effects of COVID beyond four weeks, you may submit a request for services.
A referral from your physician or specialized nurse practitioner (SNP) is now required to request services.
The referral or request from your physician or SPN confirming your diagnosis of long COVID and authorizing you to begin the CO-VIE rehabilitation services can be sent to the following e-mail address: readaptation.covid.cisssmo16@ssss.gouv.qc.ca.
