What to bring to your appointment:
- Health insurance card
- Up-to-date list of your medications, printed by your pharmacy (for each visit)
- Reports of any tests you have had since your last visit (e.g., EMG, MRI, CT scan).
- The completed questionnaire Votre premier rendez-vous à la clinique de la douleur.
You should expect your first visit to take about three hours.
