The CISSS de la Montérégie-Ouest provides several services to help people with a loss of autonomy stay at home for as long as possible. A needs assessment is required and is periodically reviewed.

Specimens for the pertussis test are collected at the CLSC’s Nursing Care Clinic (NCC) with a referral.

Since breastfeeding is encouraged, we offer two group meetings for future parents who want to learn more about its benefits. These meetings are organized by the CLSC de Salaberry-de-Valleyfield in collaboration with the Nourri-Source breastfeeding drop-in service.
The meetings are held at 71 rue Maden, in rooms 1, 2 and 3, on the 2nd floor, on the 2nd Thursday of the month, from 9 a.m. to 11 a.m., and on the 4th Thursday of the month, from 1:30 p.m. to 3:30 p.m.
As part of the follow-up after your baby’s birth, one of our nurses will also provide you with breastfeeding support starting at the first home visit and for as long as you need it.
In addition, community-based breastfeeding support services are provided by Nourri-Source.

A nurse or a psychosocial team is available to help you, guide you, support you and answer your questions. At any stage of your disease, they will be able to:
- give you information about the disease, and its treatment and consequences;
- refer you to other healthcare professionals or community resources that could better meet your needs;
- help you organize the care and services offered to you;
- support you in coping with the disease.
Consultation, support and follow-up
From the time you’re diagnosed with cancer or told that your cancer is back, the pivot nurse at your treatment centre will be there to guide you through your treatments and give you the services you need. After that, you will be transferred to our care teams for follow-up.

Nos cliniques de développement de la petite enfance offrent des services interdisciplinaires pour évaluer votre enfant, identifier ses difficultés, intervenir à court terme et l’orienter vers une ressource adaptée à ses besoins.
Composée d’ergothérapeutes, d’orthophonistes, de physiothérapeutes, d'un psychologue, de travailleurs sociaux et d'un pédiatre, notre clinique vous aide aussi à stimuler votre enfant.

SIDEP services (integrated screening and prevention services for HIV and other sexually transmitted and blood-borne infections) are free and confidential. They are provided by a nurse clinician at various community organizations, social gathering places, and CLSCs.
You can also consult services SIDEP+ that are intended EXCLUSIVELY for men having sex with other men.

What is an intellectual disability?
Intellectual disability is a disability characterized by significant limitations both in intellectual functioning and in adaptive behaviour, which covers a range of everyday social and practical skills. The impairments appear before age 18.
What is autism spectrum disorder?
Autism spectrum disorder is a neurodevelopmental disorder that significantly affects social interaction, communication and behaviour. It also manifests in the form of atypical behaviours and the development of narrow or extreme interests in specific topics. People with ASD are a highly heterogeneous group; depending on the type of impairment, they will need different types of support.
To obtain a summary or specific part of your medical record, you must:

Nutrition services are provided by nutritionists and dietetic technicians to young people under age 18 or pregnant women.
Nutrition counselling and assessments are intended to help the target population adopt healthier eating habits in order to improve or maintain an optimal level of health.
The following are the main reasons people seek out nutrition counselling:
• Children or young people under age 18 who need a diet due to a health problem (failure to thrive, obesity, food allergy or intolerance, food refusal, or gastrointestinal tract problem) or who need advice about following a vegan or vegetarian diet;
• Young pregnant woman in a difficult situation due to age or finances: OLO nutritional support program (eggs, milk, orange juice);
• Prenatal and postnatal follow-up for vulnerable clientele;
• Pregnant women with a health problem: gestational diabetes, low weight gain, high cholesterol, high blood pressure;
• Prenatal meetings: food component;
• Counselling in schools to help children develop healthy eating habits;
• Counselling in community organizations;
Group or individual counselling and follow-up are offered and are predominantly interdisciplinary.

Clientèle jeunesse
Des services de réadaptation physique, dont l'orthophonie, l'ergothérapie et la physiothérapie sont offerts au CISSS de la Montérégie-Ouest pour la clientèle jeunesse.

A nurse will provide you with follow-up tailored to your needs and those of your baby:
- breastfeeding support;
- advice on your baby’s development;
- advice on your baby’s care and feeding;
- referral for a psychosocial intervention;
- referral to a community resource.

You and your spouse can register for a series of prenatal classes led by a nurse and a nutrition technician.
Each series of four classes will cover topics of importance to expectant parents:
- nutrition during pregnancy;
- pregnancy and daily life;
- physical and psychological changes;
- the many facets of breastfeeding;
- labour and delivery;
- the new family unit, caring for the newborn, and postpartum services.
The sessions are offered every two months, six times a year. Classes take place on Tuesdays from 7 p.m. to 9 p.m.

Types of problems managed by the general social services team
- Adjustment problems or disorders related to life events or stages (e.g., bereavement, illness, separation, financial, family or marital problems, etc.)
- Crisis situation
- Symptoms of anxiety, depression and/or obsessive-compulsive disorder
- Relational problems (the intervention targets the psychosocial factors that impact the person’s life)
- Behavioural problems and their consequences (e.g., aggression, addiction, etc.)

Go to a Régie de l'assurance maladie du Québec outlet with all the required documents, including the renewal notice for your health insurance card.
If you don’t have a driver’s license
If the renewal period expired less than six months ago and you have an appointment with a doctor in the next few days, go to your local CLSC to have your application authenticated; you must bring two pieces of identification and a recent photo that meets the RAMQ criteria.

Our chronic obstructive pulmonary diseases (COPD) clinic and our asthma education centre can help you learn to live and cope with a lung disease, such as bronchitis, emphysema or asthma. Our professionals will teach you breathing techniques and follow up on all aspects related to your treatments and medications.
With a referral from a doctor, we also provide respiratory function tests to diagnose, screen for and assess lung diseases.

We offer vaccination services for children aged 2 months to 5 years. Our vaccines cover diphtheria, pertussis, tetanus, poliomyelitis, serious Hib infections, hepatitis B, rotavirus, pneumococcus, measles, rubella, mumps, type C meningitis, influenza and chickenpox.

There are several people who can help you at your school, including the nurse, social worker, specialized educator, psychologist, and remedial teacher, among others.
Youth centres
There are several youth centres in your region. You’ll meet other young people there who are just like you—new friends you can talk to and confide in. You’ll also find people who are always willing to listen if you feel the need to open up.
This service is available in schools between September and June.
