When you arrive at the hospital, you or a family member must report to the Admission office. If your health condition does not allow it, a member of the Admission office will contact your support person in order to fill out your administrative file.
Where: Emergency department registration desk
Schedule:
- Monday to Friday, from 6:45 a.m. to midnight
- Saturday, Sunday and holidays: from 8 a.m. to midnight
- Outside these hours, patients are admitted through the Emergency department.
You must have with you:
- Your valid health insurance card (keep this card with you throughout your hospital stay) and your hospital card, if you already have one. If not, we will issue you one.
- Name of your insurance company and policy number if it covers a private or semi-private room.
- List of medications and natural products you are taking. Ask your pharmacist for an up-to-date list of your medications.
- All documents issued by the doctor who referred you: request for a consultation with another doctor, requisition, results of previous examinations, etc.
- Your citizenship card, your landed immigrant card, or any other document issued by Immigration, Refugees and Citizenship Canada if you were born outside of Canada.
