If your child was vaccinated outside Québec, and you do not have a Québec vaccination booklet (the blue booklet usually given at the hospital), and you would like to update your child’s vaccination record, please make photocopies of the pages of the vaccination booklet.
Then:
Option 1 — Drop off in person
Bring the documents to the Saint‑Jean‑sur‑Richelieu Local Service Point, located at:
365, rue Normand, office 01,
from 8 a.m. to 12 p.m. and 1 p.m. to 4 p.m., Monday to Friday.
OR
Option 2 — Send by email
Scan or photograph the pages of the vaccination booklet and send them to:

Include the following information:
- Child’s first and last name
- Date of birth
- Child’s health insurance number
- Child’s sex
- Mother’s first and last name
- Father’s first and last name
- Two valid phone numbers
- Full address
- Country of origin
We will contact you when the booklet is ready. Processing time is typically between 2 and 6 weeks.
