General information

Vaccination and sampling services are offered at the local point of service in Candiac.

Description

You must have your health insurance card and your valid requisition form. Any requisition form that is not identified, not signed, incorrectly filled out, or not from Québec will be refused.

If your requisition is urgent and no time slot is available, you can call 1-833-699-2414 from 8 a.m. to 4 p.m., Monday to Friday, and we will assess your situation.

Note that some tests are not available at the Candiac local point of service. We recommend checking your requisition carefully before making an appoinment. If one of the following tests is mentioned on your requisition, please make an appointment at your CLSC.

  • Glucose challenge test with 50 g and 75 g glucose
  • 24-hour urine collection

Colorectal cancer (screening)


Colorectal cancer screening is now available at local points of service, without a referral.

If you are between the ages of 50 and 74 and meet the eligibility criteria, you can now book an appointment on the Clic Santé platform or by calling our appointment centre at 1-877-817-5279.
